This form is to be completed for any proposed changes to systems, applications, infrastructure, or services that may impact internal operations or customers. The purpose of this form is to assess the scope, risks, and resource requirements of the change, and to ensure appropriate planning, testing, and approvals are in place before implementation.

All sections must be completed with sufficient detail to enable informed decision-making by the Change Manager and, where required, the Change Advisory Board (CAB).
